  3. Pulmonary embolism โ€” assess clinical risk and associated features
  4. Pneumothorax โ€” consider if pleuritic pain or unilateral reduced breath sounds

Immediate investigations

  • 12-lead ECG promptly
  • Serial high-sensitivity cardiac troponin where available
  • Repeat vital signs and focused cardiovascular/respiratory examination
  • Additional targeted tests based on the differential and local availability

Red flags

  • Haemodynamic instability
  • New neurological deficit or syncope
  • Severe hypoxaemia or respiratory distress
  • New arrhythmia
  • Persistent or recurrent severe chest pain

Low-resource pathway

If advanced testing is unavailable, prioritise clinical assessment, serial ECG/vitals where feasible, early referral or transfer, and clear escalation criteria.
