High-risk ischemic chest pain pattern with cardiovascular risk factors. The demo prioritises ACS while screening for alternative life-threatening diagnoses.

Aortic dissection — assess for tearing pain, pulse/BP differential and neurological features
Pulmonary embolism — assess clinical risk and associated features
Pneumothorax — consider if pleuritic pain or unilateral reduced breath sounds

12-lead ECG promptly
Serial high-sensitivity cardiac troponin where available
Repeat vital signs and focused cardiovascular/respiratory examination
Additional targeted tests based on the differential and local availability

If advanced testing is unavailable, prioritise clinical assessment, serial ECG/vitals where feasible, early referral or transfer, and clear escalation criteria.
